These Forms can only be completed on a computer (not a phone), saved to your desktop and emailed or faxed to me - if you are a local MI Client and do not have a computer/laptop you can complete a hard copy when you come for a face-to-face appointment.

•  If you call me, please leave a message and a good time to call you back. I get many spam calls, so I often send calls to voicemail.

~ Grief Support sessions (locally, as well as for Midwestern and Eastern US states) will be by Zoom Online Video sessions. I also have a new Okemos office with some face-to-face appointment times available as well.
